Community Habilitation

Community Habilitation

The Community Habilitation program provides In-home services geared at teaching individuals of all ages valuable life skills and independent living skills. Services are also offered to residential based adults who prefer community habilitation over traditional day habilitation.

Direct Support Professionals work with individuals on goals such as:
  • Community Inclusion
  • Money Management
  • Situational Safety
  • Housekeeping
  • Use of Public Transportation
  • Conflict resolution and communication
  • Boundaries and relationships
  • Self-Advocacy
  • Time Management
  • Personal Hygiene
  • Diet and Exercise
  • Civic responsibilities and volunteerism
To be eligible, the individual must be:
  • Office for People with Developmental Disabilities (OPWDD) eligible
  • Have Medicaid and a Care Coordinator
  • Live in the Metro NY area
  • Be waivered (HCBS) or be eligible to be waivered
