Without Walls Day Habilitation
Day Habilitation without Walls is designed for young adults with disabilities seeking a personalized, community-based day habilitation experience. Enrolled individuals work closely with a direct support professional who will help to integrate them into their local community through various activities. All the activities are based on the interests of the individual being served and their family. These life skills and social activities may include (but are not limited to) volunteer work, attending a gym, learning basic money management, developing travel plans, community outings and participating in recreational sports such as bowling. This is a community-based day program for adults who live both within the community and in residential settings who are looking for daily community-based activities and an opportunity to work on their goals. Transportation is provided to and from the program.
To be eligible, the individual must be:
- Office for People with Developmental Disabilities (OPWDD) eligible
- Have Medicaid and a Care Coordinator
- Live in the Metro NY area
- Be waivered (HCBS) or be eligible to be waivered
- Aged out of Highschool
